THC-O acetate (often abbreviated as THC-O or ATHC) is a semi-synthetic THC derivative created by acetylation of the hydroxyl group of the THC molecule. It is among the strongest commercially available cannabinoids of the recent wave – and at the same time among the most discussed in terms of safety.

What is THC-O?

THC-O acetate practically does not occur in natural cannabis. It is produced in the laboratory by reacting classical Δ8-THC or Δ9-THC with acetic anhydride, creating an acetate 'pro-drug' form of the molecule. THC-O itself is almost inactive at the CB1 receptor – it changes into its active form only through metabolism in the human organism. This is why the onset of effect is slower for both oral and inhalation use and why the resulting intoxication intensity is described by users as 2–3× stronger than classical THC. The raw material for most commercially sold THC-O was semi-synthetic Δ8-THC produced from CBD, so the entire process involved two laboratory transformations.

In the USA THC-O acetate appeared in 2020–2022 and briefly flooded the cannabis product market. In Europe it was always a marginal matter. The most serious concern was the formation of ketene when heating acetates – the same chemically related mechanisms were behind the so-called 'vape lung' (EVALI) epidemic in the USA in 2019. Safety data on long-term use of THC-O is still missing, clinical studies have not been published.

Effects and mechanism

Users describe THC-O effects as exceptionally strong, often with a highly psychedelic character (visual distortion, deep introspection). After inhalation the effect kicks in within 15–30 minutes, orally even after 90 minutes – this latency leads to poor dosing and a risk of acute intoxication. Duration is usually 4–6 hours. Among the described side effects are strong tachycardia, paranoid states, panic attacks, disorientation and short-term psychotic episodes in predisposed individuals. With inhaled forms the main risk is the aforementioned ketene – cases of lung damage in users of THC-O vape pens have been documented internationally.

Legality of THC-O in the Czech Republic 2026

In the Czech Republic THC-O acetate is classified among prohibited psychoactive cannabinoids under the amendment to Act No. 167/1998 Coll. Distribution, sale and possession for the purpose of distribution are punishable. THC-O has the same status in most EU countries – France, Germany, Belgium, Austria and the Netherlands have explicitly banned it. In the USA the DEA stated in 2023 that THC-O acetate cannot be considered a legal cannabis derivative under the Farm Bill, in practice classifying it among controlled substances.
